Job Description
SUMMARY Coordinate intake and services for chronically homeless individuals to provide housing assistance and co occurring treatment. Provide liaison services to the local shelters, housing authorities, and hospitals to outreach individuals who are chronically homeless. Provide oversight and support to case managers, SA specialist, and nurse on the Home Base team. Oversee budgets and productivity targets. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. -Establish representation in area shelters and hospitals as liaison between agencies on an ongoing basis -Work closely with Navigators, Social Security, and DHS to ensure eligible clients are applying for benefits. -Maintain up-to-date knowledge of Medicaid insurances. -Ensure clients have access to prescribed Medication -Coordination with BHDDH for court ordered treatment for consumers – including court ordered appearances -Coordinate all shelter and hospital discharge planning for consumers. -Assistance with consumer placement with housing from shelters and hospitals. -Complete monthly census. -Assist manager in overseeing services and treatment provided by case managers and other staff on Home Base Team Assist manager in ensuring proper documentation is completed by team members in accordance with HIPPA, BHDDH, TPC and CARF requirements. -Ensure staff are meeting targets for HH, RISOC, and productivity. -Oversee budget, donations, and petty cash for team. Supervise the daily operations of the team. -Provide leadership to staff regarding training, risk management, day-to-day matters, etc. Facilitate an environment that supports staff participation and encourages self direction and team building. -Assist manager in Facilitating team meetings for the development, review and implementation of treatment plans, in a timely manner. -Develop staff schedules to assure on-call and adequate coverage to meet consumer needs. -Provide orientation and training to new staff. -Assist with ongoing training and staff development. -Conduct record audits of staff records in accordance with Center requirements. -Establish plans of improvement as needed. -Serve as a member of the CSS Management Team to develop programs, in-service trainings, solve problems, generate ideas and to implement Center policies. Carry a small caseload and provide direct service. -Treat consumers with respect. -Provide support counseling, problem-solving, contracts and limit setting. -Establish attainable goals with the consumer. -Involve consumers, agencies and others in planning. -Assist consumers in deciding which areas they want to address. -Assist consumers with basic medical procedures and hygiene needs. -Encourage consumers to attain the highest possible levels of independence. -Accept the values and lifestyles of consumers. -Use a flexible approach with a diverse population. -Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with substance abusers, as it relates to case management. -Present verbally a synopsis of actual cases as required. -Arrange moves for consumers. -Accompany consumers to appointments with agencies, clinics, doctors, etc. -Coordinate a variety of services. Make assessments and provide appropriate crisis interventions and risk management as needed. -Assess consumers' unmet needs (psychological, physical, housing, psychiatric). -Identify, assess and respond to crisis situations. -Perform in a calm and professional manner during crisis or emergency situations. -Teach consumers adult daily living skills. Maintain cooperative relationships with Center staff, consumers, community agencies and the public. -Communicate with various local agencies and other departments or teams at The Center. -Work in cooperation with co-workers and supervisory staff. -Deal with team members regarding problems in a constructive manner. -Accept and use direction and supervision. -Benefit from constructive criticism. -Attend mandatory in-service trainings and other trainings required. OCCASIONAL DUTIES: -Serve on appropriate Center committees.
Required Education
Bachelor’s degree in Human Services field. Minimum of 2 years related experience in direct care with the psychiatrically disabled population and/or in a similar social service agency. Minimum 2 years prior supervisory experience preferred. Must have valid driver's license and insured auto.
