Affinity Primary Care East Side is located on the Butler Hospital campus on the beautiful east side of Providence, RI. The practice is a newly integrated behavioral primary care medicine practice, affiliated with Care New England, which includes Butler Hospital, Kent Hospital,and Women & Infants Hospital. The primary care team includes physicians, behavioral health consultants and medical assistants. The practice supports medical students and the graduate medical education programs at The Warren Alpert Medical School of Brown University, for training internists and psychiatrists. It is also a training site for post-doctoral fellows in clinical psychology.

Edgar A. Pollak, MD
Edgar A. Pollak was born and raised, and still resides in the suburbs of Boston, MA. He attended Holy Cross College as an undergraduate, where he earned a bachelor's degree in biology, and earned his medical degree at Tufts University School of Medicine in Boston. Dr. Pollak completed his residency in internal medicine at St. Joseph's Hospital and Medical Center in Phoenix, AZ in 2007. He has been board certified in internal medicine since 2007 and recently became board certified in addiction medicine in 2015. Dr. Pollak sees a wide spectrum of patients, ranging from healthy young adults to the geriatric population. He also holds a special interest in aiding patients addicted to substances achieve sobriety through counseling and proper medication. Dr. Pollak believes in providing quick access to his patients when they are sick or injured and will do his best to make appointments available in an appropriate amount of time to help patients avoid unnecessary trips to urgent care centers or emergency rooms. Dr. Pollak is currently accepting new patients.
Daniel Evans, PhD
Daniel Evans, PhD, is a psychology post-doctoral fellow. He has experience helping primary care patients cope with a range of behavioral health concerns including anxiety, depression, obesity, smoking, and medication adherence. He also has an interest in helping patients better manage chronic diseases like diabetes, heart disease, and chronic pain.
