Job Description
SUMMARY Provide psycho-social rehab services in order to facilitate program goals and objectives in a quality care approach. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Provide case management and support to consumers. -Treat consumers with respect. -Provide support counseling, problem solving, contracts and limit setting. -Establish attainable goals with the consumer. -Involve consumer, agencies and others in planning. -Assist consumer in deciding which areas they want to address. -Assist and monitor the group home consumers with basic medical procedures and hygiene needs. -Encourage consumers to attain the highest possible levels of independence. -Accept the values and lifestyles of consumers. -Use a flexible approach with a diverse population. Make assessments and provide appropriate crisis interventions and risk management as needed to maintain a calm and safe environment. -Treat consumers with respect. -Assess consumers' unmet needs (psychological, physical, housing, psychiatric). -Identify, assess and respond to crisis situations. -Use a flexible approach with a diverse population. Teach consumers adult daily living skills assisting them in all aspects of their life in the group home. -Teach, supervise and assist consumers with doing household chores (morning and evening, laundry, room cleaning, etc.) using realistic situations. -Use a step-by-step process when teaching skills. -Use positive reinforcement and encouragement. -Integrate skills training in all aspects of Rehab. Plan. -Complete chores if consumers' skill level warrants or when consumer refuses. -Maintain a clean, healthy and safe environment at all times. -Perform household duties as needed. -Use a flexible approach with a diverse population. Initiate treatment and advocate for consumer within network of care. -Negotiate and schedule appointments for consumers within The Center and with outside agencies (Social Security Supplemental Insurance (S.S.I.), Food Stamps, Advocates Office, Medical Doctor, etc.). -Complete required interagency and other necessary paperwork. Negotiate on behalf of the consumer, as needed, with landlords, courts, families, treatment team, consumers themselves. -Negotiate with reasonable goals. -Clarify the issues to be negotiated. -Follow through with the agreement reached. -Remain flexible and consider alternatives. -Advocate for consumer. Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities. -Develop professional relationship with Day Program staff. -Provide and maintain a home atmosphere. -Maintain and promote family contacts and relationships. Transport consumers in own vehicle or Center van to planned activities and appointments. -Drive carefully and abide by traffic rules. -Ensure that seat belts are worn by all traveling in Center owned vehicles. Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy. -Complete daily activity logs. -Complete progress notes with appropriate format. -Develop and implement a plan of care with ongoing consumer input. -Complete a clear, specific, behaviorally oriented treatment plan as required, with consumer signature. -Monitor confidentiality in accordance with Center policy and legal requirements. -Complete required reports and evaluations (MHPRR checklist, Psycho-social form). -Follow required office procedures in a cooperative manner. -Initiate and process incident reports as per Center guidelines. -Adhere to Infection Control protocols. Maintain cooperative relationships with Center staff, consumers, community agencies and the public. -Communicate with various local agencies and other departments or teams at The Center. -Work in cooperation with co-workers and supervisory staff. -Deal with team members regarding problems in a constructive manner. -Accepts and uses direction and supervision. -Benefits from constructive criticism. Attend trainings, case presentations and conferences. -Provide feedback to staff on the content of educational programs attended. -Attend mandatory in-service trainings and other trainings required. OCCASIONAL DUTIES: -Serve on appropriate Center committees. -Work flexible hours, including evenings, weekends, holidays and during inclement weather.
Required Education
Associates degree in Human Services or related field required. Bachelor’s degree preferred. Experience in residential facilities helpful. Must have valid driver's license and insured auto.
