And your information is confidential: Only you can access it, unless you choose to give permission to another person.

Personal Health Record (PHR)
You can track your own and your family’s health information and history, including flu and tetanus shots, other immunizations, medications, surgeries, allergies, and more. All of the information you record is confidential and is available only to you.
- You may give permission to access your information to someone else, such as a family member, friend, or health care professional.
- You may also print a copy to take with you to a doctor’s appointment or on a vacation.

Condition Centers
Four Centers are available to help people manage these common health conditions: Asthma, Congestive Heart Failure (CHF), Diabetes, and Sleep Disorders.
- Diabetes Center: You can enter your glucometer reading into your PHR simply by connecting it to your computer and clicking on Medical Devices, if you have a commonly used glucometer.

This section allows you to keep your Personal Health Record and to access a Medication Center, where you can research medications, side effects, and dosing.

The Medication Center
The Medication Center lets you research medications, side effects and dosing. You may also store information about the medications you are currently taking. If you enter a new medication, any interactions that may occur between medications you have stored in this section will automatically be checked.
Reference
If you click on “Reference,” you will find:
- Health Information on diseases, injuries, surgical procedures, and much more
- Drug Leaflets for information on medications (prescribed or over-the-counter
- Drug Interactions for information on medication interactions (prescribed or over-the-counter)
You can research medications, side effects and dosing. This section also allows you to store information about the medications you are currently taking. When you enter a new medication into this Center, any interactions that may occur between medications you have stored here and that new medication(s) will automatically be checked.
Account
Clicking on “Account” will bring you to Member Maintenance and Permission Maintenance sections where you can:
- Create accounts for family members and manage your password through Member Maintenance
- Give access to a family member, friend or clinician by logging on and clicking on “Permission Maintenance.”
