Job Description
The Lung Program nurse navigator is integral to the success of the Program's mission to provide outstanding and coordinated care for our patients and families. The nurse navigator must have a highly visible role within the Lung Program and will help customize the role as the Lung Program develops. The primary responsibility of the nurse navigator is to coordinate the care experience of the most complex Lung Cancer Program patients.  The navigator will work in partnership with the clinical team and practice leaders toward the primary goal of ensuring the effective delivery of care to Lung Program patients.

Coordinate care and serve as an advocate for the most complex Lung Cancer Program patients.
Develop and operationalize clinical pathways for the assessment and care coordination of Lung Cancer Program patients.
Develop and lead efforts to measure the quality and safety of patient care and evaluate patient outcomes.
Develop and review Lung Cancer Program patient-facing education materials and communications.

RI RN licensure

At least 5 years of clinical experience in a medical office or hospital setting required.  Direct clinical experience with pulmonary or thoracic surgery patients and knowledge of their disease processes and treatments required.  Must work independently and collaboratively and have sound decision making skills.
