Job Description
SUMMARY Provide psycho-social rehab services in order to facilitate program goals and objectives in a quality care approach. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Provide case management and support to consumers. -Treat consumers with respect. -Provide support counseling, problem solving, contracts and limit setting. -Establish attainable goals with the consumer. -Involve consumer, agencies and others in planning. -Assist consumer in deciding which areas they want to address. -Encourage consumers to attain the highest possible levels of independence. -Accept the values and lifestyles of consumers. -Use a flexible approach with a diverse population. Initiate treatment and advocate for consumer. -Discharge planning and referrals. -Running Groups -Negotiate on behalf of the consumer, as needed, with landlords, courts, families, treatment team, consumers themselves. Assist in the development and implementation of socialization and recreational activities within the group home, in the community, and by attending Day Program groups and activities. -Develop professional relationship with Program staff. -Provide and maintain a safe atmosphere. -Maintain and promote family contacts and relationships. Transport consumers in own vehicle or Center van to planned discharge appointments. -Drive carefully and abide by traffic rules. -Ensure that seat belts are worn by all traveling in Center owned vehicles. Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements and abide by Center confidentiality policy. -Complete required interagency and other necessary paperwork. -Complete daily activity logs. -Complete progress notes with appropriate format. -Complete a clear, specific, behaviorally oriented treatment plan as required, with consumer signature. -Monitor confidentiality in accordance with Center policy and legal requirements. -Complete required reports and evaluations. -Follow required office procedures in a cooperative manner. -Initiate and process incident reports as per Center guidelines. Maintain cooperative relationships with Center staff, consumers, community agencies and the public. -Communicate with various local agencies and other departments or teams at The Center. -Deal with team members regarding problems in a constructive manner. -Accept and use direction and supervision. -Benefit from constructive criticism. Attend trainings, case presentations and conferences as required. OCCASIONAL DUTIES: -Serve on appropriate Center committees. -Work flexible hours, including inclement weather or state of emergencies as the CSU is a 24/7 program
Required Education
Bachelor's degree required. Experience in residential or acute facilities helpful. Must have valid driver's license and insured auto. Must be able to work flexible hours as well as cover the program when unforeseen circumstances arise to ensure 24-hour coverage.
