Manages the referral process out of the inpatient and emergency department settings, including booking appointments ordered at discharge from the inpatient and emergency department settings. Assures resources are in place for the patient and helping to increase care retention within the system.
Associates degree required. Medical Assistant training program preferred. Must have excellent customer service skills, computer proficiency, flexibility, team player, ability to grow/learn in role. Bilingual in Spanish preferred.
