The Program Manager, Population Health is responsible for the development, implementation, and management of transformative and operational initiatives and programs directed at enhancing the performance of the Integra Community Care Network ACO. By partnering with clinical, operational, and financial colleagues across business lines to execute on assigned initiatives, this position will be a major contributor to the success of the Integra ACO. Will lead efforts towards performance improvement by excelling across many dimensions, but most importantly through: developing and project managing new and existing operational initiatives to drive performance, drawing insights from data modeling and ad-hoc analyses to generate and implement high-value recommendations for new programs and being a reliable knowledge expert on evolving national and local population health performance improvement.
Bachelors’ degree required; MBA, MHA, MSW or graduate nursing degree preferred. Five to seven years working in a fast-paced health care environment. Healthcare provider and/or payer experience required; ACO, Medicare, Medicaid population health experience strongly preferred. Understanding of the health care delivery setting required. Clinical experience preferred. Proficiency in Microsoft Office applications – Word, Excel, Project, PowerPoint, Visio. Experience leading change and driving program development. Advanced problem solving skills. Strong interpersonal skills and ability to interact positively with a wide range of constituents. Ability to work both independently, within a team environment and a multi-dimensional environment. Creative, flexible, self-motivated, must possess sound judgment ability to plan and initiate new activities consistent with achieving service excellence. Excellent written and verbal communication skills. Ability to present information to different size audiences. Strong organizational and project management skills including development of project parameters, goals, and timelines and outcome measurement. Passion for working towards improving the health of populations. Analytic skills to collect information from diverse sources and summarize the information and data in order to solve problems. Ability to function effectively within an ever-changing environment and to meet deadlines and reprioritize as necessary. Must be able to maintain confidentiality of information. Willing to travel when needed.
