37.5 hours per week Monday-Friday 8am-4pm
The Palliative Care Nurse Practitioner will provide medical home visits to patients with advanced illness and function as part of the VNA Palliative Care Team. The Palliative Care Nurse Practitioner will support the VNA Hospice by providing face to face visits. The Palliative Care Nurse Practitioner will ensure compliance with established professional and accreditation standards, practices and regulatory requirements.
EDUCATION: Advance Practice Nurse from an accredited university. EXPERIENCE: At least one (1) years previous experience providing Palliative Care. Must possess the ability to coordinate a high level of activity under a variety of conditions and constraints. LANGUAGE SKILLS: Ability to create documents such as medical records, medical reports, etc. Ability to write routine reports and correspondence. Must be able to speak, read, and write English with excellent oral and written communication skills required. REASONING ABILITY: Ability to define problems, collect data, establish facts and draw valid conclusions. Ability to interpret an extensive variety of technical instructions furnished in written, oral, diagram or schedule form. Must be able to deal with several abstract and concrete variables. OTHER SKILLS AND ABILITIES Position requires showing initiative, flexibility and good judgment. Must be able to work independently and complete assignments with minimal supervision. Must display attention to detail. Must work effectively with others, help others solve problems, and promote teamwork. Excellent organizational skills require, with the ability to multi-task. Knowledge of computer systems and patient information required. CERTIFICATES, LICENSES AND REGISTRATIONS RI NPP License, DEA license and RI prescriptive substance provider license.
